Complaint Form
If you are not satisfied with a service provided by a health, disability or mental health service provider, or you are concerned with the health, conduct or performance of a registered or unregistered health practitioner, it is your right to make a complaint. Before making a complaint to us, please try talking with the service provider as this is often the quickest and easiest way to address your concerns or resolve the problem.
If you need help completing our complaints form, please contact us. All complaints must be confirmed in writing.
You can also contact us to check we received your form or discuss the status of your complaint.
We have four ways you can complete our Complaint Form:

1. Directly on this website:
Please gather all of the information you will need to complete the form before you start, this will make it easier to complete. You may find it useful to download the printed form as a guide to what information you will need.

2. Fillable form:
This is the same as the printable complaints form but with fields you can fill in. It also has a digital signature option so you don't have to print it off and sign it.
- Download the form and save it onto your computer. Please Note: you must open the form from your download folder as a document or the digital signature will not work.
- Fill in all the fields as much as you can, remember to save as you go
- Sign the form using the digital signature
- Save the form and send it to us with any attachments (such as a letter or email from the service provider) by email to: mail@hadsco.wa.gov.au
If you're not using your own computer, or find it hard to set up the digital signature, you might prefer to use the website online form above.
If you have filled in the fillable form and can't get the digital signature to work, then you can always just print it and sign it. If you have saved it, then all the details you have filled in should print out too.

4. By Post:
You can contact us and ask for a complaint form to be sent to you in the post. Once filled in and signed, please return it to us.
